In order to upgrade the Sanguinarix in Lords of the Fallen, you will need to talk to Pieta at Skyrest. She will offer you the chance to upgrade the Sanguinarix, as long as you have a Saintly Quintessence. You can earn Saintly Quintessence by beating bosses, although sometimes it is dropped in the boss arena, not by the boss directly.

The Sanguinarix cannot be upgraded until after you have faced the first main boss of the game, Pieta, She of Blessed Renewal. After this fight, she will dive off the side of the bridge and the path opens for you to go down into Skyrest, a safe area full of NPCs to chat where you will find Pieta.
