Smith competed domestically as an amateur for Rotunda ABC in Liverpool, where he won the Great Britain Middleweight and Welterweight titles in 2010 and 2011, as well as international recognition for GB Boxing.

Smith continued the family tradition at the 2010 Commonwealth Games in New Delhi, India, where he won a silver medal in the welterweight division. His brothers Paul and Stephen had previously won gold and silver medals at the Melbourne, Australia, 2006 Commonwealth Games, and the 2002 Commonwealth Games in Manchester, England, respectively.

On September 21, 2013, Callum Smith won the vacant English super-middleweight title by stopping Patrick Mendy in the first round, setting a British record.

The WBC International super-middleweight title was vacated after Smith defeated Ruben Eduardo Acosta (27-8, 10 KOs) via sixth-round knockout on October 26.

Smith won the WBC International championship back on May 17 by knocking out Tobias Webb in the second round. On July 12, Smith then triumphed over 32-year-old southpaw Vladine Biosse by unanimous decision.

At the Echo Arena on April 2, 2016, Smith squared off against French boxer Hadillah Mohoumadi (20-3-1, 15 KOs). This was a final eliminator for the European super-middleweight title as well as the WBC super-middleweight belt. Smith prevailed through first-round TKO.

On May 16, Smith was added to the undercard of Tony Bellew’s May 29 world title fight at Goodison Park, along with lightweight Sean Dodd and heavyweight David Price.

Smith then competed on the undercard of Gennady Golovkin vs. Kell Brook, the world middleweight championship bout, which took place at the O2 Arena on September 10, 2016.

Smith declared he would compete once more before the year ended, hoping for a shot at the world title. The fight would take place on the undercard of Anthony Joshua vs. Éric Molina on 10 December at the Manchester Arena.

The WBC ordered the start of discussions for a match between Smith and WBC champion Badou Jack (20-1–3, 12 KOs) on January 16, 2017. This eliminated James DeGale’s chance for a unification rematch, but Jack also said he intended to advance to light-heavyweight.

During the draft Gala held at the renowned Grimaldi Forum in Monaco, second-seeded Smith was selected to face Swedish opponent Erik Skoglund (26–0, 12 KOs).

It was confirmed on February 13 that the super-middleweight tournament’s championship match would take place on June 2, 2018, in London’s O2 Arena.

Following the victories of Smith against Holzken on February 24 and George Groves (28–3, 20 KOs) over Chris Eubank Jr. on February 17, the match was rescheduled.

In June 2019, Smith squared up against Hassan N’Dam N’Jikam, the WBA’s 13th-ranked super middleweight contender, on the undercard of Anthony Joshua vs. Andy Ruiz Jr.

On November 23, 2019, domestic opponent John Ryder, ranked No. 1 by the WBA and No. 4 by the WBC, was the opponent Smith faced in his second defense of his WBA and The Ring super-middleweight belts.

Smith attempted to defend his super middleweight championships at WBA and The Ring against Canelo Álvarez for the third time on December 19, 2020.

On September 25, 2021, Smith made his debut in the light-heavyweight division against former WBA championship contender Lenin Castillo, following his first professional loss to Canelo Álvarez. The bout was part of the undercard of Anthony Joshua vs. Oleksandr Usyk.

Castillo was carried out of the ring on a stretcher after Smith brutally knocked him out in the second round. Smith and Bauderlique squared up in a WBC world title eliminator on August 20, 2022, the undercard of Anthony Joshua II vs. Oleksandr Usyk.

In the seventh round, Artur Beterbiev stopped Callum Smith in Quebec City, Canada, on January 13, 2024.
